All initial appointments:

If this is your first appointment we would like you to complete an exercise pre-screening form to help us prepare for your consultation. You can complete the form online, download the pdf and send it back via email or bring it with you to your appointment.  For telehealth consultations, as we are not in the room with you we need to make sure you are safe as possible or have someone nearby to help. In the event that something happens we need to know your location and who is with you at your location in case we need to send help. These details can be provided in the prescreening form.

Send through your referral form
All telehealth services with a valid referral are bulk billed through Medicare. You must send your referral through to us before the appointment. It may also be helpful for us to send through a copy of your care plan or team care arrangement if possible. You can do this by:

• Attached through a third-party application like CareMonitor

• Posted to us: PO Box 3095 Mount Druitt Village, NSW 2770 (please allow enough time for this to be received and picked up from the PO box)
